Advance ID and Crystal Reports Server Security Request

Information Services provides Advance and Crystal Reports Server ID's to associates on the Fayetteville campus of the University of Arkansas with approved security. To request an ID, please complete a "Request for Advance ID Form" and sign.

This form must be approved by your supervisor. In addition, anyone requesting access to donor records MUST sign the statement of confidentiality.

Users should then return the form to:
